At any age, a medical crisis could leave someone too ill to express his or her own healthcare wishes. This can make it difficult for family, friends, and healthcare providers to know what best to do in difficult and likely irreversible situations.
A written Advance Directive allows a person to express his or her preferences in such situations and to designate a family member or friend to make medical decisions if the individual is incapacitated.
POLST (Physician Orders for Life-Sustaining Treatment) is another document that allows the individual to specify what he or she wants for end-of-life medical care.
