Participants will hear from experts in the field with presentations on advanced care planning, nutrition, how to handle the unexpected life challenges of aging, and understanding geriatric frailty.

Attendees can also discover community resources and will have an opportunity to visit with and ask questions of exhibitors who specialize in planning for this next stage in life.

Presenters:

Dr. Daniel Hoefer is the chief medical officer for Sharp HealthCare's Outpatient Palliative Care Program (Transitions) and is the associate medical director for Sharp HospiceCare. He is board certified in family and palliative medicine and is affiliated with Sharp Rees-Stealy Medical Group.

Dr. Dara Schwartz is a lead clinical psychologist for the Senior Intensive Outpatient Program at Sharp Mesa Vista Hospital.

John Tastad is a spiritual care counselor who coordinates the Advance Care Planning Program at Sharp HospiceCare.

Tracy Childs and Ami Chitalia are nutritional consultants with Veg-Appeal.
